PROMISEC 40 MG CAPSULE 10 may interact with the following medicines:

• Antitubercular medicines (e.g., Rifampicin): Rifampicin may reduce omeprazole levels and decrease its acid-suppressing effect. Your doctor may monitor symptom control and adjust treatment if necessary.
• Antiepileptic medicines (e.g., Phenytoin): Omeprazole may increase phenytoin levels, which can increase the risk of toxicity, including dizziness, coordination problems, and involuntary eye movements. Your doctor may monitor phenytoin levels and adjust the dose if necessary.
• Antiplatelet medicines (e.g., Clopidogrel): Omeprazole may reduce the activation and effectiveness of clopidogrel, which may decrease its ability to prevent blood clots. Your doctor may consider an alternative acid-reducing medicine or monitor treatment closely.
• Anticoagulants (e.g., Warfarin): Omeprazole may increase warfarin levels and enhance its blood-thinning effect, increasing the risk of bleeding. Your doctor may monitor INR levels more frequently and adjust the dose if required.
• Antiretroviral medicines (e.g., Nelfinavir): Concomitant use with omeprazole is not recommended because omeprazole may significantly reduce nelfinavir levels and decrease its effectiveness.
• Anticancer medicines (e.g., Methotrexate): High-dose methotrexate taken with omeprazole may reduce methotrexate elimination from the body, increasing the risk of toxicity. Your doctor may temporarily stop omeprazole or monitor treatment closely.

PROMISEC 40 MG CAPSULE 10 should be used with caution if you have:

• Osteoporosis: Long-term use of omeprazole may reduce calcium absorption and increase the risk of fractures of the hip, wrist, or spine, particularly in older adults and patients with osteoporosis. Your doctor may recommend calcium and vitamin D supplementation and monitor bone health during prolonged treatment.
• Vitamin B12 Deficiency: Long-term suppression of stomach acid may reduce vitamin B12 absorption, which may worsen existing vitamin B12 deficiency and lead to symptoms such as fatigue, weakness, numbness, or tingling sensations. Your doctor may monitor vitamin B12 levels and recommend supplementation if necessary.
• Hypomagnesaemia (Low Magnesium Levels): Prolonged use of omeprazole may further lower magnesium levels, potentially causing muscle cramps, weakness, tremors, seizures, or irregular heart rhythms. Your doctor may periodically monitor magnesium levels during long-term treatment.
• Systemic Lupus Erythematosus (SLE): Proton pump inhibitors have rarely been associated with worsening of lupus symptoms or the development of cutaneous lupus erythematosus. Patients with lupus should inform their doctor before starting treatment and report any new skin rash or joint symptoms.
• Gastric Cancer: Omeprazole may temporarily relieve symptoms of stomach cancer and delay diagnosis. Patients with unexplained weight loss, difficulty swallowing, recurrent vomiting, gastrointestinal bleeding, or persistent stomach pain should undergo appropriate evaluation before starting long-term treatment.
• Chronic Diarrhoea or Clostridioides difficile Infection: Long-term acid suppression may increase the risk of gastrointestinal infections, including Clostridioides difficile-associated diarrhoea. Patients with persistent diarrhoea should consult their doctor promptly, who may investigate the cause and review treatment.

The daily dose of PROMISEC 40 MG CAPSULE 10 should be taken as prescribed by your doctor. The dosage may vary depending on the condition being treated, its severity, and your response to treatment. Do not change the dose without consulting your doctor.

Taking more than the recommended dose may cause symptoms such as nausea, vomiting, dizziness, headache, drowsiness, or abdominal discomfort. Consult your doctor immediately or seek medical attention in case of an overdose.

If you forget to take a dose of PROMISEC 40 MG CAPSULE 10, take it as soon as you remember. However, if it is almost time for your next scheduled dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed dose.
