Blastomycosis in both pulmonary and extrapulmonary forms
Histoplasmosis, including chronic cavitary pulmonary disease and disseminated non-meningeal histoplasmosis
Pulmonary and extrapulmonary aspergillosis, particularly in patients who cannot tolerate amphotericin B therapy
Onychomycosis, involving both toenails and fingernails, caused by dermatophytes (tinea unguium)
