Uses of Linagliptin + Metformin

  • Linagliptin + Metformin is used as a combination therapy for the management of Type 2 Diabetes along with diet and exercise.
  • This combination functions to augment postprandial insulin secretion and diminish endogenous glucose production in the body.

How Linagliptin + Metformin works

  • Metformin exerts its effects through multiple mechanisms, including the reduction of hepatic glucose production, inhibition of intestinal glucose absorption, and enhancement of insulin sensitivity through increased peripheral glucose uptake and utilisation. Significantly, compared to sulfonylureas (SUs), metformin does not induce hypoglycemia. 
  • Linagliptin is an inhibitor of DPP-4, an enzyme that breaks down incretin hormones, namely glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). Consequently, linagliptin elevates the concentrations of active incretin hormones, promoting insulin release in a glucose-dependent manner and reducing circulating glucagon levels. GLP-1 and GIP play vital roles in the physiological regulation of glucose homeostasis. Incretin hormones are continuously secreted at basal levels throughout the day, with levels surging immediately after meal consumption. GLP-1 and GIP enhance insulin biosynthesis and secretion from pancreatic beta cells, particularly in the presence of normal and elevated blood glucose levels. Additionally, GLP-1 diminishes glucagon secretion from pancreatic alpha cells, leading to a decrease in hepatic glucose output.

Overdose and Missed Dose

  • Overdose of this combination can cause lactic acidosis, such as feeling cold or uncomfortable, severe nausea or vomiting, stomach ache, unexplained weight loss, muscular cramps, or rapid breathing. Inform your doctor regarding these effects and follow the advice accordingly.
  • If you forget a dose of this combination, take it as soon as you remember. But avoid repeating the dose.

Use In Driving And Operating Machinery

  • This combination does not directly impact your ability to drive or operate machinery. However, they may lead to hypoglycemia which can cause dizziness. Taking precautions to prevent low blood sugar while driving or handling machinery is essential.

Use in Pregnancy and Lactation

  • Linagliptin + Metformin is not recommended in pregnant and breastfeeding women.

Special Dietary & Lifestyle Recommendation

  • Making lifestyle changes, including dietary modifications and regular exercise, is essential when taking Linagliptin + Metformin to control blood sugar levels properly.
  • After initiating treatment with Linagliptin + Metformin, it's recommended to have your blood sugar levels reassessed after 15 days and, subsequently, at regular intervals every 3-6 months. This monitoring helps track the progress of the disease and allows for dose adjustments as needed.
  • If hypoglycemia occurs, it's essential to have a quick source of sugar readily available. Carry sugar candies, glucose tablets, or sugary foods and drinks without fat or protein, such as fruit juice, to promptly address low blood sugar episodes.
