Uses of Metformin + Sitagliptin

Metformin + Sitagliptin is a combination antidiabetic medicine used to help control blood sugar levels in adults with type 2 diabetes.

This combination is commonly used for:

Type 2 diabetes management: Helps lower elevated blood sugar levels when diet, exercise, and a single antidiabetic medicine are not providing adequate control.
Postprandial (after-meal) blood sugar control: Helps reduce the sharp rise in blood sugar that occurs after eating by enhancing the body's natural insulin response.

How Metformin + Sitagliptin works

  • Metformin + Sitagliptin works through two complementary mechanisms that together help lower blood sugar levels in people with type 2 diabetes.
  • Metformin primarily works by reducing the amount of glucose produced by the liver, decreasing the absorption of glucose from the intestine, and improving the body's sensitivity to insulin, allowing cells to take up and use glucose more effectively. Unlike some other antidiabetic medicines, Metformin does not significantly increase the risk of hypoglycaemia (low blood sugar) when used alone.
  • Sitagliptin belongs to a class of medicines called DPP-4 inhibitors. It works by blocking the enzyme dipeptidyl peptidase-4 (DPP-4), which normally breaks down hormones called incretins (GLP-1 and GIP) released by the intestine after eating. By preventing the breakdown of these incretin hormones, Sitagliptin allows them to remain active for longer. This increases insulin release from the pancreas when blood sugar is high and reduces the release of glucagon, a hormone that raises blood sugar, in a glucose-dependent manner.
  • Together, Metformin reduces the underlying glucose supply while Sitagliptin enhances the body's own insulin response after meals, providing more comprehensive blood sugar control.

  • It is not recommended to use any antidepressants (bupropion), glaucoma medications, antibiotics (cephalexin, ciprofloxacin), antacids (cimetidine), heart medications (digoxin), anti-HIV medications (dolutegravir), alcoholic drinks, saliva-thinning medications (glycopyrrolate), iodinated X-ray contrast agents, anti-epileptic medications (topiramate, lamotrigine), and chest pain medications (ranolazine). Sex hormones (androgens), anti-oxidants (alpha-lipoic acid), pain relievers (aspirin), anti-tuberculosis medicines (prothionamide), growth hormones (pegvisomant), and other antidiabetic medications. Before using any prescription or over-the-counter medications, you must consult your doctor.

Overdose and Missed Dose

  • An overdose of Metformin + Sitagliptin can lead to a rare but serious condition called lactic acidosis. Symptoms may include feeling unusually cold or uncomfortable, severe nausea or vomiting, stomach pain, severe weakness or tiredness, muscle cramps, or rapid breathing. If you experience any of these symptoms, inform your doctor immediately and seek urgent medical attention.
  • If you forget to take a dose of this combination, take it as soon as you remember. If it is nearly time for your next dose, skip the missed dose and continue with your regular schedule. Do not take a double dose to compensate for the missed one.

Use In Driving And Operating Machinery

  • Metformin + Sitagliptin itself does not directly impact your ability to drive or operate machinery. However, it may lead to hypoglycemia which can cause dizziness. Taking precautions to prevent low blood sugar while driving or handling machinery is essential.

Use in Pregnancy and Lactation

  • Metformin + Sitagliptin is not recommended in pregnant and breastfeeding women.

Special Dietary & Lifestyle Recommendation

  • Making lifestyle changes, including dietary modifications and regular exercise, is essential when taking Metformin + Sitagliptin to control blood sugar levels properly.
  • After initiating treatment with Metformin + Sitagliptin, it's recommended to have your blood sugar levels reassessed after 15 days and, subsequently, at regular intervals every 3-6 months. This monitoring helps track the progress of the disease and allows for dose adjustments as needed.
  • If hypoglycemia occurs, it's essential to have a quick source of sugar readily available. Carry sugar candies, glucose tablets, or sugary foods and drinks without fat or protein, such as fruit juice, to promptly address low blood sugar episodes.
