Uses of Metformin + Sitagliptin

  • Metformin + Sitagliptin is used as a combination therapy for the management of Type 2 Diabetes along with diet and exercise.
  • This combination functions to augment postprandial insulin secretion and diminish endogenous glucose production in the body.

How Metformin + Sitagliptin works

  • Metformin exerts its effects through multiple mechanisms, including the reduction of hepatic glucose production, inhibition of intestinal glucose absorption, and enhancement of insulin sensitivity through increased peripheral glucose uptake and utilisation. Compared to sulfonylureas (SUs), metformin does not induce hypoglycemia significantly. 
  • Sitagliptin functions as a dipeptidyl peptidase-4 (DPP-4) inhibitor in patients with type 2 diabetes, exerting its effects by retarding the inactivation of incretin hormones. The drug elevates concentrations of active intact incretin hormones, specifically glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP), enhancing and extending their physiological actions. Incretin hormones are endogenously released by the intestine, with increased levels following a meal. DPP-4 is an enzyme that rapidly inactivates these hormones. GLP-1 and GIP play a role in the regulation of glucose homeostasis. In the presence of normal or high blood glucose levels, GLP-1 and GIP stimulate insulin synthesis and release from pancreatic cells through intracellular signaling pathways involving cyclic AMP. GLP-1 also suppresses glucagon secretion from pancreatic alpha cells, reducing hepatic glucose production. By prolonging and increasing active incretin levels, sitagliptin enhances insulin release and reduces circulating glucagon levels in a glucose-dependent manner.

  • It is not recommended to use any antidepressants (bupropion), glaucoma medications, antibiotics (cephalexin, ciprofloxacin), antacids (cimetidine), heart medications (digoxin), anti-HIV medications (dolutegravir), alcoholic drinks, saliva-thinning medications (glycopyrrolate), iodinated X-ray contrast agents, anti-epileptic medications (topiramate, lamotrigine), and chest pain medications (ranolazine). Sex hormones (androgens), anti-oxidants (alpha-lipoic acid), pain relievers (aspirin), anti-tuberculosis medicines (prothionamide), growth hormones (pegvisomant), and other antidiabetic medications. Before using any prescription or over-the-counter medications, you must consult your doctor.

Overdose and Missed Dose

  • Overdose of Metformin + Sitagliptin can cause lactic acidosis, such as feeling cold or uncomfortable, severe nausea or vomiting, stomach ache, unexplained weight loss, muscular cramps, or rapid breathing. Inform your doctor regarding these effects and follow the advice accordingly.
  • If you forget to take a dose of this combination, take it as soon as you remember. But avoid repeating the dose.

Use In Driving And Operating Machinery

  • Metformin + Sitagliptin itself does not directly impact your ability to drive or operate machinery. However, it may lead to hypoglycemia which can cause dizziness. Taking precautions to prevent low blood sugar while driving or handling machinery is essential.

Use in Pregnancy and Lactation

  • Metformin + Sitagliptin is not recommended in pregnant and breastfeeding women.

Special Dietary & Lifestyle Recommendation

  • Making lifestyle changes, including dietary modifications and regular exercise, is essential when taking Metformin + Sitagliptin to control blood sugar levels properly.
  • After initiating treatment with Metformin + Sitagliptin, it's recommended to have your blood sugar levels reassessed after 15 days and, subsequently, at regular intervals every 3-6 months. This monitoring helps track the progress of the disease and allows for dose adjustments as needed.
  • If hypoglycemia occurs, it's essential to have a quick source of sugar readily available. Carry sugar candies, glucose tablets, or sugary foods and drinks without fat or protein, such as fruit juice, to promptly address low blood sugar episodes.
