  • Vomiting
  • Diarrhoea
  • Abdominal pain

How Azithromycin + Fluconazole + Secnidazole works

  • Azithromycin + Fluconazole + Secnidazole is a combination of an antibiotic, an antifungal, and an antiprotozoal agent.
  • Azithromycin stops bacterial growth by inhibiting essential protein synthesis within bacterial cells.
  • Fluconazole prevents the formation of ergosterol, an essential component of fungal cell membranes, disrupting fungal growth.
  • Secnidazole damages the DNA and energy production of protozoan organisms, killing susceptible protozoa and anaerobic bacteria.
  • Together, these three active agents act against susceptible bacterial, fungal and protozoal causes of mixed urogenital infection.

Expert Advice

Dosage Forms Available

  • Azithromycin + Fluconazole + Secnidazole is commonly available as an oral tablet kit. In many products, the kit may contain 1 tablet of azithromycin, 1 tablet of fluconazole, and 2 tablets of secnidazole. The strength of each medicine and the way they should be taken may vary depending on the product. Follow the instructions on the pack and your doctor’s advice.

Age and Dose Restrictions

  • This combination kit is generally used in adults. In individuals below 18 years of age, it should be used only when specifically prescribed by a doctor.
  • Whether all the tablets should be taken together or at different times depends on the product and the doctor’s instructions. Do not decide the dose yourself.

Contraindications

  • Do not take this medicine if you are allergic to azithromycin, fluconazole, secnidazole, or any other ingredient in the kit.
  • If you have previously developed a serious liver problem after taking azithromycin, do not take this medicine without consulting your doctor.
  • If you are taking medicines such as erythromycin, pimozide, or quinidine, inform your doctor before starting this kit. Taking these medicines with fluconazole may cause serious effects on heart rhythm.
  • Do not self-treat with this kit during pregnancy.

Other Medications to Avoid

  • This combination may increase the risk of abnormal heart rhythm when taken with medicines that can affect heart rhythm, such as amiodarone and quinidine.
  • Taking it with blood thinners such as warfarin may increase the risk of bleeding.
  • Blood sugar may become too low when it is taken with diabetes medicines such as glimepiride, glipizide, or glyburide.
  • It may affect the effects of medicines such as phenytoin, used for epilepsy, and rifampicin, used to treat certain infections.
  • A time gap may be required between taking aluminium- or magnesium-containing antacids and azithromycin.
  • Inform your doctor or pharmacist about all the medicines, vitamins, or herbal products you take. Do not stop any medicine without medical advice.

Overdose and Missed Dose

  • Taking more than the recommended amount may cause severe nausea, vomiting, diarrhoea, dizziness, hearing loss, confusion, liver problems, or abnormal heart rhythm. Seek immediate medical attention if an overdose is suspected.
  • This medicine is generally given as a kit or for a short treatment period. If a tablet or dose is missed, ask your doctor or pharmacist when to take it. Do not take a double dose to make up for a missed dose.

Management of Side Effects

  • Mild nausea, abdominal pain, or changes in taste may improve with time. If you experience stomach discomfort, ask your doctor or pharmacist whether the medicine should be taken with food.
  • Drink sufficient water and other fluids to prevent dehydration if you have diarrhoea or vomiting.
  • Do not stop the antibiotic or the treatment course without medical advice. However, if you develop a severe allergic reaction, severe skin rash, difficulty breathing, or swelling of the face or throat, stop the medicine and seek immediate medical attention.
  • Persistent or bloody diarrhoea, severe abdominal pain, yellowing of the skin or eyes, or dark-coloured urine may indicate a serious problem. Contact your doctor immediately if these symptoms occur.

Use in Driving and Operating Machinery

  • Some people may experience dizziness, headache, or difficulty concentrating with this combination. Avoid driving or operating heavy machinery if you experience such symptoms.

Use in Pregnancy and Lactation

  • Do not use this combination during pregnancy without the specific advice of a gynaecologist. Oral fluconazole is generally not used for vaginal yeast infections during pregnancy. Your doctor may prescribe a more appropriate medicine during pregnancy.
  • If you are breastfeeding, inform your doctor before taking this medicine. After taking secnidazole, your doctor may advise you to stop breastfeeding for approximately 4 days and discard the breast milk during this period. Follow the instructions provided with your product and your doctor’s advice.

Caution in Other Conditions

  • Inform your doctor if you have liver or kidney disease, irregular heart rhythm, heart failure, low potassium or magnesium levels, a condition causing muscle weakness, diabetes, or a bleeding disorder.
  • Also inform your doctor if you have previously experienced a severe allergic reaction or severe skin reaction after taking an antibiotic or antifungal medicine.

Special Dietary and Lifestyle Recommendations

  • Wear loose, cotton underwear and change wet clothes promptly to prevent moisture from accumulating.
  • Do not wash inside the vagina with water or any other substance. Fragranced soaps, sprays, and vaginal cleansing products may increase irritation.
  • Avoid alcohol and alcohol-containing products during treatment with secnidazole and for at least 2 days after taking it.
  • If trichomoniasis or a sexually transmitted infection is suspected, sexual partners may also need testing and treatment. Avoid sexual activity until treatment is completed and symptoms have resolved, or follow your doctor’s advice regarding condom use.
  • Drink sufficient water and maintain a balanced diet. Staying hydrated and making dietary changes do not replace the treatment prescribed by your doctor.

Disease/Condition Glossary

  • Bacterial Vaginosis: Bacterial vaginosis (BV) is a infection in the vagina caused by an imbalance in the good bacteria, where harmful bacteria overgrow and outnumber beneficial ones. When BV becomes severe, it can lead to a strong fishy odour, greyish-white discharge, itching, and irritation. BV is diagnosed through a pelvic exam, vaginal swab tests, and measuring vaginal pH (typically higher than usual in BV). Treatment often involves antibiotics to restore balance and prevent recurrence.
  • Vaginal Yeast Infection: A vaginal yeast infection or candidiasis, is caused by the overgrowth of Candida fungi. It leads to itching, burning, thick white discharge, and irritation. However, frequent or untreated infections, although not usually dangerous, can cause persistent discomfort and may signal an underlying issue, such as diabetes or immune suppression. Diagnosis involves a pelvic exam and microscopic examination of vaginal discharge. Treatment typically includes antifungal creams, tablets, or suppositories to relieve symptoms and restore balance.
  • Trichomoniasis: Trichomoniasis is a sexually transmitted infection (STI) caused by the Trichomonas vaginalis. It often causes vaginal itching, burning, redness, frothy yellow-green discharge, and an unpleasant odor.
