Uses of Secnidazole

Secnidazole is a 5-nitroimidazole antibiotic and antiprotozoal medicine used to treat bacterial and parasitic infections, particularly those affecting the digestive and reproductive systems.

Secnidazole is commonly used for:

• Bacterial vaginosis: Treats abnormal overgrowth of anaerobic bacteria in the vagina, causing abnormal discharge, odour, and discomfort. A single-dose regimen is often used.
• Trichomoniasis: Eliminates infection caused by the sexually transmitted parasite Trichomonas vaginalis, causing vaginal itching and discharge.
• Giardiasis: Treats intestinal infection by Giardia lamblia, characterised by watery diarrhoea, bloating, and abdominal pain.
• Amoebiasis (amoebic dysentery): Manages intestinal and invasive infections caused by Entamoeba histolytica.
• Anaerobic bacterial infections: Treats infections caused by anaerobic bacteria in various body sites including the abdomen and pelvis.

How Secnidazole works

  • Secnidazole works by entering bacterial and parasitic cells and causing damage to their DNA, preventing reproduction and leading to the death of the microorganism.
  • It is a 5-nitroimidazole antimicrobial and acts as a prodrug, meaning it must be chemically activated inside the target cell to exert its effect.
  • Inside susceptible anaerobic bacteria and protozoa, the nitro group of Secnidazole is reduced by the organism's own electron transport system (ferredoxin-type redox pathway). This reduction produces a highly reactive metabolite a nitro radical anion that:
    • Causes strand breaks in microbial DNA, disrupting its structure and preventing replication.
    • Inhibits nucleic acid synthesis, blocking the production of RNA and DNA that the microorganism needs to grow and multiply.
  • Because human cells lack the ferredoxin-based electron transport system, they cannot activate Secnidazole in this way. This selective activation means Secnidazole targets microorganisms specifically without damaging human cells at therapeutic doses.
  • Secnidazole has a longer half-life than older nitroimidazoles such as Metronidazole, allowing effective treatment with fewer doses.
