Uses of Sacubitril + Valsartan

Sacubitril + Valsartan is a combination medicine used in adults with chronic heart failure to reduce the risk of cardiovascular death and hospitalisation due to heart failure.

This combination is commonly used for:

  • Chronic heart failure with reduced ejection fraction: Helps reduce the risk of hospitalisation and manage symptoms such as breathlessness and fatigue.
  • Reducing cardiovascular death risk: Supports heart function and lowers the risk of death related to heart failure.
  • Improving exercise tolerance: Helps patients manage daily activities with reduced breathlessness and tiredness.
  • Fluid retention associated with heart failure: Assists in managing symptoms such as swelling in the legs and ankles, as part of overall heart failure management.

How Sacubitril + Valsartan works

  • Sacubitril + Valsartan combines two medicines that work together to support heart function.
  • Sacubitril blocks an enzyme called neprilysin, which normally breaks down natural substances in the body that help widen blood vessels and remove excess sodium and fluid. By inhibiting this enzyme, Sacubitril allows these beneficial substances to remain active for longer.
  • Valsartan is an angiotensin receptor blocker (ARB) that blocks the action of angiotensin II, a hormone that narrows blood vessels and raises blood pressure. Together, these two components widen blood vessels, reduce strain on the heart, and help the body eliminate excess fluid, easing the workload on a weakened heart.

Dosage Forms Available

  • Sacubitril + Valsartan can be taken orally (tablet).

Age And Dose Restrictions 

  • This combination is intended to be used by adults and children above 1 year. 

Contraindications 

  • This combination is contraindicated in patients with a history of hypersensitivity to sacubitril or valsartan. It is also contraindicated in patients with a history of angioedema related to previous ACE inhibitors (ACEi) and angiotensin II receptor blockers (ARBs) therapy.
  • It is also contraindicated for the concomitant use of aliskiren in diabetic patients.

Medications To Avoid With This Drug

  • Potassium-sparing diuretics, such as amiloride and spironolactone, may increase blood potassium levels.
  • NSAIDs (such as ibuprofen and naproxen) may increase the risk of impaired kidney function.
  • Taking lithium with this medicine may increase the risk of lithium toxicity.
  • Potassium-containing salt substitutes should not be used without medical advice.

Overdose And Missed Dose

  • Overdose can cause hypotension. If you miss a dose of this combination, take it as soon as you remember. But, if the next dose is near, skip the missed dose and resume the regular dosing regime.

Management Of Side Effects

  • If you experience hypotension or dizziness, get out of bed slowly, resting your feet on the floor for a few minutes before you stand. If side effects worsen or persist, consult your doctor for a possible treatment plan adjustment.

Use In Driving And Operating Machinery

  • This combination can make you feel dizzy and lightheaded. It is not recommended to drive, operate machinery, or do other activities requiring mental alertness or coordination after this combination. 

Use In Pregnancy And Lactation

  • This combination is not recommended during pregnancy and breastfeeding.

Caution In Other Conditions

  • Caution is advised in elderly patients and patients with hypotension (low blood pressure), impaired kidney function or impaired liver function.
  • This medicine may cause angioedema (deep swelling under the skin). Seek immediate medical attention if you develop swelling of the face, lips, tongue or throat, or difficulty breathing.

  • Avoid alcohol consumption, as it may increase the risk of adverse side effects.
  • Adopt a low-sodium diet to help manage blood pressure. Limiting salt intake is crucial for individuals taking this combination to optimise their cardiovascular health.
  • Strictly adhere to the prescribed medication regimen. Consistent and timely intake of this combination is crucial for managing chronic heart failure and reducing associated risks.
