Uses of Tolvaptan

  • Tolvaptan is a vasopressin receptor antagonist used to correct low blood sodium levels (hyponatraemia) and to slow kidney function decline in certain kidney conditions.
  • Tolvaptan is commonly used for:

    1. Hyponatraemia (low blood sodium): Raises dangerously low sodium levels in the blood associated with conditions such as heart failure, liver cirrhosis, and syndrome of inappropriate antidiuretic hormone secretion (SIADH).
    2. Heart failure-associated hyponatraemia: Helps correct sodium imbalance in patients with heart failure where fluid retention contributes to low sodium levels.
    3. Liver cirrhosis-associated hyponatraemia: Used to manage low sodium in patients with advanced liver disease and fluid accumulation (ascites).
    4. SIADH (Syndrome of Inappropriate Antidiuretic Hormone): Corrects persistent low sodium caused by excess water retention due to abnormal hormone activity.
    5. Autosomal dominant polycystic kidney disease (ADPKD): Slows the growth of kidney cysts and the decline in kidney function in adults at risk of rapidly progressing ADPKD.

How Tolvaptan works

  • Tolvaptan works by blocking the action of a hormone called vasopressin (also known as antidiuretic hormone or ADH) in the kidneys.
  • Vasopressin normally signals the kidneys to reabsorb water back into the bloodstream, reducing urine output. In conditions such as heart failure, cirrhosis, and SIADH, vasopressin activity becomes excessive, causing too much water to be retained in the body. This dilutes the sodium in the blood, leading to hyponatraemia.
  • Tolvaptan selectively blocks vasopressin V2 receptors in the kidney's collecting ducts, preventing water reabsorption. As a result, the kidneys excrete more free water in the urine a process called aquaresis without significant loss of electrolytes such as sodium or potassium. This helps gradually restore normal sodium levels in the blood.
  • In ADPKD, blocking V2 receptors also reduces the hormonal signalling that drives cyst growth in the kidneys, helping to slow the progression of the disease.

Symptoms may include confusion, difficulty speaking, or movement problems. If an overdose is suspected, seek emergency medical attention immediately. This medicine requires careful dose monitoring, particularly in a hospital setting.
  • If you miss a dose of Tolvaptan, take it as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and continue with your regular schedule. Do not take two doses at the same time to make up for a missed one.

Management of Side Effects

Most side effects of Tolvaptan are temporary and generally harmless, resolving when the medicine is discontinued. However, if you experience any severe side effects or worsening of symptoms, please consult your doctor.
