Tofacitinib

Uses of Tofacitinib

Tofacitinib is a Janus kinase (JAK) inhibitor used to manage symptoms and control inflammation in certain autoimmune and inflammatory conditions.

Tofacitinib is commonly used for:

  • Rheumatoid arthritis: Reduces joint pain, swelling, and stiffness and helps slow progression of joint damage.
  • Psoriatic arthritis: Helps reduce joint inflammation, pain, and stiffness associated with psoriatic arthritis.
  • Ulcerative colitis: Helps control inflammation of the colon and rectum, reducing symptoms such as abdominal pain and diarrhoea.
  • Ankylosing spondylitis: May be used in adults with active disease who have not responded adequately to, or cannot tolerate, certain TNF-blocking treatments.

Side effects of Tofacitinib

  • Upper respiratory tract infections
  • Headache
  • Diarrhoea
  • Nasopharyngitis
  • Increased blood cholesterol levels

How Tofacitinib works

  • Tofacitinib works by blocking enzymes called Janus kinases (JAKs), particularly JAK1 and JAK3, which are involved in triggering inflammation inside cells.
  • These enzymes normally relay signals from cytokines and growth factors, chemical messengers that drive the inflammatory response in autoimmune conditions.
  • By inhibiting JAK1 and JAK3, Tofacitinib interrupts these signalling pathways, reducing the inflammation that causes joint damage, pain, and swelling in conditions such as rheumatoid arthritis and psoriatic arthritis, and the gut inflammation seen in ulcerative colitis.

Expert Advice

Dosage forms available

  • Tofacitinib can be taken orally (tablets and extended-release tablets).

Age and dose restrictions

  • Tofacitinib is used in adults and, for certain conditions, in children 2 years of age and older.

Contraindications

  • Tofacitinib should not be used in patients with severe liver problems. Lymphocyte, neutrophil, and haemoglobin levels should be checked before starting treatment, and treatment should not be initiated if these levels are very low. It should also not be used in patients with an active, serious infection.

Other medications to avoid

  • Tofacitinib may interact with strong CYP3A4 inhibitors, such as ketoconazole, itraconazole, and clarithromycin. Rifampicin is a strong CYP3A4 inducer and may reduce the effect of Tofacitinib.

Overdose and missed dose

  • Overdosing on Tofacitinib could potentially lead to severe side effects or complications. If you suspect an overdose, seek immediate medical attention.
  • In case you miss a dose of Tofacitinib, take it as soon as you remember. However, if it is nearly time for your next dose, skip the missed dose and continue your regular dosing schedule.

Management of side effects

  • Most side effects are temporary and typically resolve once the medication is discontinued. However, if you experience severe side effects or worsening symptoms, consult your doctor immediately.
  • Drinking plenty of water may help manage some side effects such as diarrhoea or nausea. It can also help keep you hydrated if you experience fever or excessive sweating due to an infection.

Use in driving and operating machinery

  • Tofacitinib may cause dizziness or fatigue in some individuals. If you experience these side effects, avoid driving or operating heavy machinery until you feel alert and well enough to do so safely.

Use in pregnancy and lactation

  • This medicine is not recommended during pregnancy and breastfeeding unless recommended by your doctor. Consult your doctor before using this medicine during pregnancy and breastfeeding.

Caution in other conditions

  • Patients with kidney disease or mild to moderate liver problems should exercise caution when using Tofacitinib. The doctor may need to adjust the dose in these conditions.

Special dietary and lifestyle recommendations

  • Regular hand washing and avoiding close contact with people who have infections can reduce the risk of catching infections while on this medication, as Tofacitinib can potentially lower your body's ability to fight off infections.
  • Regular check-ups and blood tests as advised by your doctor are vital for monitoring your response to treatment and any potential side effects.

Frequently asked questions

No, Tofacitinib is not a steroid. It belongs to a group of medicines known as Janus Kinase Inhibitors (JAK inhibitors).

Tofacitinib may cause fatigue in some patients, which could potentially affect your sleep. However, it is not widely reported to directly influence sleep patterns.

Tofacitinib is not directly associated with affecting human fertility. If you have concerns about this, please consult your doctor.

Individual responses may vary and any significant changes should be discussed with your doctor.

Yes, it is safe to take Tofacitinib daily as long as it is prescribed by your doctor. Do not alter or stop your dosage without consulting your doctor.

The time taken for Tofacitinib to show an effect may vary depending on the individual’s condition and the treatment being given. Some people may begin to experience an improvement in symptoms within a few weeks.

Tofacitinib can be taken with or without food. The time at which it should be taken should be as advised by your doctor.

Tofacitinib is used to manage autoimmune conditions such as rheumatoid arthritis, psoriatic arthritis, and ulcerative colitis. It works by reducing the overactivity of the immune system, which helps decrease inflammation, relieve pain and swelling, and improve joint function. It is usually prescribed when other treatments have not worked well.
