Contraindications

• Isosorbide-5-mononitrate is contraindicated in patients with severe anemia, hypotension (low blood pressure), increased intracranial pressure, or allergy to nitrates. It should not be used in individuals with a recent history of stroke or heart attack unless recommended by a doctor.

Other Medications to Avoid

• Avoid using Isosorbide-5-mononitrate with medications such as sildenafil (Viagra), tadalafil (Cialis), and vardenafil (Levitra), as these can cause a severe drop in blood pressure. Other medications to avoid include certain antihypertensives, alcohol, and ergot alkaloids. Always inform your doctor about any other medications you are taking to prevent potential interactions.

Overdose and Missed Dose

• An overdose of Isosorbide-5-mononitrate can cause severe hypotension, dizziness, fainting, and palpitations. In such cases, immediate medical attention is necessary. 
• If a dose is missed, take it as soon as you remember. However, if it is almost time for the next dose, skip the missed dose and continue with the regular schedule. Do not take a double dose to make up for the missed one.

Management of Side Effects

• Headache is a common side effect of Isosorbide-5-mononitrate. To manage this, you can use mild pain relievers like paracetamol after consulting your doctor. If dizziness or low blood pressure occurs, sit or lie down immediately until symptoms improve. Staying hydrated and avoiding sudden movements can help reduce dizziness. If side effects persist or worsen, seek medical advice.

Use in Driving and Operating Machinery

• Isosorbide-5-mononitrate may cause dizziness, lightheadedness, or blurred vision. If affected, avoid driving, operating machinery, or engaging in activities that require alertness until you know how the medication affects you.

Use in Pregnancy and Lactation

• The use of Isosorbide-5-mononitrate during pregnancy should be based on a doctor’s recommendation, as its safety has not been fully established. It should be used with caution in breastfeeding mothers, as it is not known whether the medication passes into breast milk.

Caution in Other Conditions

• Caution is advised in individuals with low blood pressure, dehydration, kidney disease, liver disease, or thyroid disorders. Patients with a history of migraines or glaucoma should also consult their doctor before starting this medication.
