Icosapent Ethyl

Uses of Icosapent Ethyl

  • Icosapent Ethyl is a highly purified form of eicosapentaenoic acid (EPA), an omega-3 fatty acid, used to lower dangerously high triglyceride levels and reduce the risk of serious cardiovascular events in high-risk adults.
  • Icosapent Ethyl is commonly used for:

    1. Severe hypertriglyceridaemia: Reduces significantly elevated triglyceride levels (500 mg/dL or above) in adults, as an adjunct to diet, when dietary changes alone are insufficient.
    2. Cardiovascular risk reduction: Lowers the risk of major adverse cardiovascular events such as heart attack, stroke, and cardiovascular death in adults with elevated triglyceride levels who also have established cardiovascular disease or type 2 diabetes with additional risk factors.
    3. Adjunct to statin therapy: Used alongside statin medicines in patients whose triglyceride levels remain elevated despite adequate cholesterol-lowering treatment.

Side effects of Icosapent Ethyl

  • Joint pain (arthralgia)

  • Muscle pain

  • Peripheral oedema (swelling of the legs/ankles)

  • Constipation

  • Gout

How Icosapent Ethyl works

  • Icosapent Ethyl works by reducing the amount of triglycerides produced and circulating in the bloodstream, through several complementary actions.
  • It reduces the liver's production of triglycerides (fats) and enhances their clearance from the bloodstream by improving the activity of enzymes that break down fat particles in the blood. This leads to a significant lowering of triglyceride levels, particularly in patients with very high baseline levels.
  • Beyond its triglyceride-lowering effect, Icosapent Ethyl as a purified EPA omega-3 has been shown to reduce inflammation in blood vessel walls, stabilise the fatty plaques that build up in arteries, and reduce platelet aggregation (the tendency of blood cells to clump together and form clots). These combined actions are thought to contribute to its cardiovascular protective benefits, which go beyond simply lowering blood fat levels.
  • Unlike some other omega-3 products, Icosapent Ethyl does not contain DHA (docosahexaenoic acid), which has been associated with increases in LDL cholesterol in some patients. This makes it a preferred option in patients where LDL management is also a concern.

  • It is important that you discuss all your current medications with your doctor to prevent potential harmful interactions.

Overdose and Missed Dose

  • Taking more than the prescribed dose of Icosapent Ethyl may cause symptoms such as abdominal pain, nausea, or diarrhoea. If an overdose is suspected, seek immediate medical attention. There is no specific antidote, and treatment will be supportive based on the symptoms present.
  • If you miss a dose of Icosapent Ethyl, take it as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and continue with your regular schedule. Do not take two doses at the same time to make up for a missed one.

Management of Side Effects

Most side effects are temporary and generally harmless and resolve when discontinuing this medicine. However, if you experience any severe side effects or worsening of any of the symptoms, please consult your doctor.

  • If experiencing muscle or joint pain, try taking your dose with a larger meal or closer to bedtime.

  • For those experiencing constipation, increasing water intake may help alleviate this issue.

Use in Driving and Operating Machinery

  • Taking Icosapent Ethyl does not typically impair your ability to drive or operate machinery. However, monitoring how it affects your body individually is advisable before engaging in such activities.

Use in Pregnancy and Lactation

  • The use of Icosapent Ethyl during pregnancy and lactation should be avoided unless clearly necessary. If you are pregnant or breastfeeding, consult your doctor before starting this medication.

Caution in Other Conditions

  • Patients with liver disease need to use Icosapent Ethyl cautiously. Regular liver function tests might be required to monitor your condition.

  • Similarly, those with known thyroid disorders should consult their doctor for specific advice on taking this medicine.

Special Dietary and Lifestyle Recommendations

  • Alongside taking Icosapent Ethyl, adopting a diet low in saturated fats and cholesterol can help manage high triglyceride levels effectively.

  • Incorporate more omega-3 rich foods, such as oily fish (salmon, mackerel), nuts (walnuts), and seeds (flaxseeds) into your diet.

  • Regular physical activity can also help control triglyceride levels. Aim for at least 150 minutes of moderate-intensity aerobic exercise per week.

  • Avoid high-sugar and high-fat foods which can spike triglyceride levels. Instead, opt for whole grains, lean proteins, and plenty of fruits and vegetables.

  • Managing stress through techniques like mindfulness practices or yoga may indirectly help maintain healthy triglyceride levels by reducing triggers that might lead to unhealthy eating habits or sedentary behaviour.
