Uses of Flupirtine

Flupirtine is a non-opioid analgesic used to treat acute pain, particularly musculoskeletal pain and pain associated with muscle spasm.

Flupirtine is commonly used for:

Musculoskeletal pain: Relieves pain in muscles, tendons, ligaments, and bones, including back pain, neck pain, and pain following orthopaedic procedures.
Post-operative pain: Used for short-term management of pain following surgery, including orthopaedic and gynaecological procedures.
Headache and migraine: May be used in some cases for the relief of headache pain.
Dysmenorrhoea (menstrual pain): Helps relieve cramping and lower abdominal pain during menstruation.
Cancer-related pain: May be used as part of pain management in selected patients under specialist supervision.
Fibromyalgia and chronic pain syndromes: May be used in some cases for the management of chronic pain.

How Flupirtine works

  • Flupirtine works as a centrally acting, non-opioid analgesic through a unique mechanism that distinguishes it from other pain medicines such as NSAIDs or opioids.
  • It selectively activates neuronal potassium channels (specifically KCNQ/Kv7 potassium channels) on the surface of nerve cells in the central and peripheral nervous system. When these potassium channels open, potassium ions flow out of the nerve cell, making the cell less electrically excitable. This reduces the ability of pain-transmitting nerve cells (neurons) to fire repetitively, dampening overactive pain signals.
  • This mechanism also contributes to muscle-relaxant properties: by reducing excessive nerve signalling in motor pathways, Flupirtine decreases abnormal muscle tension and spasm that often accompanies musculoskeletal pain.
  • Additionally, Flupirtine has indirect antagonism at NMDA (N-methyl-D-aspartate) receptors, which may contribute to its analgesic effects. This means it does not act through opioid receptors.

  • It is intended for adults above 18 years of age.

Contraindications 

  • It is contraindicated in patients with a history of hypersensitivity to Flupirtine.

Medications To Avoid With This Drug 

  • Avoid taking anticoagulants (warfarin), analgesics and antipyretics (paracetamol) and anticonvulsants (carbamazepine). These medications may alter the safety and effectiveness of Flupirtine.

Overdose And Missed Dose

  • An overdose of Flupirtine requires urgent medical management. Symptoms of overdose may include excessive sedation, confusion, nausea, vomiting, tachycardia (rapid heartbeat), and in severe cases, liver toxicity. If you suspect an overdose, contact emergency services or go to the nearest hospital immediately. Strict adherence to the prescribed dose is essential.
  • If you miss a dose of Flupirtine, take it as soon as you remember. If it is nearly time for your next dose, skip the missed dose and continue with your regular schedule. Do not take two doses at the same time to make up for the missed one.

Management Of Side Effects

  • Take this medicine with food to reduce gastrointestinal side effects and continue taking it as prescribed. If you experience dizziness, get out of bed slowly, resting your feet on the floor for a few minutes before you stand. If side effects worsen or persist, consult your doctor for a possible treatment plan adjustment.

  • Caution is advised during pregnancy and breastfeeding.

Caution In Other Conditions

  • Caution is advised in older adults and patients with myasthenia gravis, chronic alcoholism, and liver disease.

Special Dietary And Lifestyle Recommendation

  • Limit alcohol consumption, as excessive alcohol use can increase the risk of liver-related side effects associated with Flupirtine.
  • Perform gentle stretches to improve flexibility and prevent muscle stiffness. Avoid overstretching, especially if you have an injury.
  • You may apply a warm compress or ice pack to affected areas to relieve musculoskeletal pain.
  • Choose comfortable, supportive shoes to minimise the impact on joints and muscles.
