L-Methylfolate

Uses of L-Methylfolate

L-Methylfolate is the active, biologically ready form of folate (vitamin B9) used to treat folate deficiency and conditions related to low folate levels in the body.

L-Methylfolate is commonly used for:

  • Folate deficiency: Corrects low folate levels caused by inadequate dietary intake, malabsorption, or increased requirements during pregnancy.
  • Megaloblastic anaemia: Treats a type of anaemia caused by folate deficiency in which the body produces abnormally large red blood cells that cannot function properly.
  • Adjunctive treatment in depression: Used as an adjunct (add-on) treatment in patients with depression who have an inadequate response to antidepressants. Some patients may have genetic variations that affect folate metabolism.
  • Elevated homocysteine (hyperhomocysteinaemia): Helps lower raised levels of homocysteine in the blood.
  • Pregnancy support: Supports healthy fetal development during pregnancy by helping maintain adequate folate levels.
  • Cognitive support: May support normal neurological function through its role in folate-dependent metabolic processes.

Side effects of L-Methylfolate

  • Nausea

  • Vomiting

  • Diarrhoea

  • Abdominal cramps

  • Skin rash

How L-Methylfolate works

L-Methylfolate works by directly participating in essential biochemical processes in the body that require folate, without needing to be converted by the body first.

Most dietary folate and supplemental folic acid must be converted into L-Methylfolate (5-methyltetrahydrofolate) through a series of steps involving an enzyme called MTHFR (methylenetetrahydrofolate reductase). Some people have a genetic variation that reduces the efficiency of this enzyme, leaving them with lower active folate levels even when folic acid intake is sufficient.

L-Methylfolate bypasses this conversion entirely because it is already in the active form the body uses directly.

Once available, L-Methylfolate:

  • Converts homocysteine to methionine: This process helps maintain normal homocysteine metabolism and supports DNA synthesis and repair.
  • Supports neurotransmitter production: L-Methylfolate is needed to produce monoamine neurotransmitters serotonin, dopamine, and norepinephrine which play a role in normal brain function.
  • Supports red blood cell formation: Ensures proper DNA replication during the production of red blood cells in the bone marrow, helping to prevent megaloblastic anaemia associated with folate deficiency.
  • Supports fetal development: Provides folate required for normal fetal growth and development during pregnancy.

Expert Advice

Dosage forms available

  • L-Methylfolate is available for use in the form of oral tablets and capsules, which are swallowed.

  • It is also available as sublingual tablets, which are placed under the tongue to dissolve.

  • For those who may have difficulty swallowing tablets, L-Methylfolate is available in a liquid form.

  • In severe cases of folate deficiency, L-Methylfolate can be administered via an intramuscular injection.

Age and dose restrictions

  • L-Methylfolate is suitable for use by both adults and children. The medicine is available for individuals of all ages.

Contraindications

  • This medicine is contraindicated in patients with untreated vitamin B12 deficiency.

  • It is also contraindicated for individuals who have an allergy to any of the active or inactive ingredients of L-Methylfolate.

Other medications to avoid

  • This medicine may interact with anti-convulsants (such as phenytoin, carbamazepine, and valproic acid), medications used as immunosuppressant (like methotrexate), and medicines that affect intestinal absorption such as sulfasalazine.

  • When considering starting new medications, consulting a doctor is vital to avoid potential interactions.

Overdose and missed dose

  • An overdose of L-Methylfolate may cause symptoms such as nausea, abdominal discomfort, or other gastrointestinal symptoms. These symptoms are generally mild. If they occur or you suspect you have taken significantly more than prescribed, seek medical advice promptly.
  • If you miss a dose of L-Methylfolate, take it as soon as you remember. If it is nearly time for your next dose, skip the missed dose and continue with your regular schedule. Do not take two doses at the same time to compensate. Consistent daily use is particularly important during pregnancy and when managing folate-related deficiencies.

Management of side effects

Most side effects are temporary and generally harmless and resolve when discontinuing this medicine. However, if you experience any severe side effects or worsening of any of the symptoms, please consult your doctor immediately.

  • If experiencing gastrointestinal disturbances such as nausea or abdominal cramps while taking L-Methylfolate, consider taking this medicine with food or adjusting the timing of the dose closer to meal times.

  • For those experiencing insomnia linked to high doses, adjusting the timing of taking the medication might help. Taking it earlier in the day rather than close to bedtime can be beneficial.

Use in driving and operating machinery

  • L-Methylfolate does not generally affect your ability to drive or operate heavy machinery. No specific precautions are usually necessary; however, individual reactions can vary.

Use in pregnancy and lactation

  • L-Methylfolate is crucial during pregnancy to prevent neural tube defects. It should be used during pregnancy under the guidance of a doctor who can determine appropriate dosing. Safety during lactation has not been specifically contraindicated; however, consulting a doctor before use while breastfeeding is advised.

Caution in other conditions

  • Individuals with a history of allergies or hypersensitive reactions should exercise caution when taking L-Methylfolate.

  • While no specific cautions exist regarding renal or hepatic conditions, it is prudent for individuals with these health issues to consult a doctor before starting L-Methylfolate.

Special dietary and lifestyle recommendations

  • Maintain a balanced diet rich in natural folates such as dark leafy greens (spinach, kale), asparagus, broccoli, citrus fruits, beans, peas, lentils, nuts, and whole grains to support overall health while managing conditions that require L-Methylfolate.

  • Limit intake of alcohol as it can interfere with folate absorption.

  • Engage in regular cardiovascular exercise like walking or swimming which supports overall heart health especially important in managing homocysteinaemia.

  • Manage stress through relaxation techniques such as yoga or meditation which can be beneficial, especially for those using L-Methylfolate for mood regulation related to conditions like depression.

Frequently asked questions

No, L-Methylfolate is not a steroid. It is a biologically active form of folate, a B vitamin essential for various bodily functions.

L-Methylfolate is not known to have any specific effects on sleep. However, it plays a crucial role in the synthesis of neurotransmitters which do affect mood and sleep cycles.

There is no evidence to suggest that L-Methylfolate impacts fertility in either men or women.

No, specific dietary restrictions are associated with the use of L-Methylfolate. However, maintaining a healthy diet can contribute to overall well-being.

Yes, it is generally safe to take L-Methylfolate daily, as long as you follow the dosage recommended by your doctor.

The time taken for L-Methylfolate to work can vary depending on the individual and the condition being managed.

L-Methylfolate can be taken at any time of the day, but it is usually recommended to take it at the same time each day for consistency.

L-Methylfolate is the active form of folate used to treat folate deficiency and megaloblastic anaemia. It is also used as an adjunct in certain depressive disorders to support neurotransmitter synthesis.
