Uses of Itopride

Itopride is a prokinetic medicine used to relieve symptoms associated with delayed stomach emptying and upper gastrointestinal disorders.

Itopride is commonly used for:

• Functional dyspepsia: Relieves upper abdominal discomfort, bloating, a feeling of fullness after eating, nausea, and heartburn caused by slow stomach emptying.
• Symptoms of chronic gastritis: Relieves symptoms such as postprandial fullness, upper abdominal discomfort, loss of appetite, nausea, vomiting, and bloating associated with chronic gastritis.
• Gastroparesis: Assists in conditions where the stomach empties too slowly, causing persistent nausea, vomiting, and bloating.
• Nausea and vomiting: Controls nausea and vomiting related to digestive disorders or sluggish gut movement.
• Bloating and abdominal distension: Reduces the feeling of abdominal fullness and gas caused by impaired gastrointestinal motility.

How Itopride works

Itopride works by increasing the movement of food through the stomach and upper digestive tract through a dual mechanism that raises the levels of acetylcholine a chemical that stimulates gut contractions.

Itopride acts in two complementary ways:

  • Dopamine D2 receptor antagonism: Dopamine normally slows down gut movement. Itopride blocks dopamine D2 receptors in the stomach and intestines, removing this slowing effect and allowing the gut to contract more efficiently and move food forward.
  • Acetylcholinesterase inhibition: Acetylcholine is the key chemical that stimulates gut contractions. An enzyme called acetylcholinesterase (AChE) normally breaks down acetylcholine, reducing its effect. Itopride blocks this enzyme, preventing the breakdown of acetylcholine. This leads to higher acetylcholine levels, which stimulate stronger and more coordinated stomach contractions.

The combined result of these two actions is:

  • Faster gastric emptying (food moves out of the stomach more quickly)
  • Reduced bloating and nausea
  • Improved symptoms associated with reduced gastrointestinal motility
  • Improved coordination between the stomach and the duodenum (upper small intestine)

Unlike some older prokinetics, Itopride does not cross into the brain in significant amounts, reducing the risk of central nervous system side effects.

  • Itopride can be taken orally (tablet and capsule).

Age And Dose Restrictions 

  • It is intended to be used by adults above 16 years of age. 

Contraindications 

  • It is contraindicated in patients with a history of hypersensitivity to Itopride.

Other Medications To Avoid With This Medicine

  • Avoid taking anticholinergic medications (tiquizium bromide, scopolamine butyl bromide, and timepidium bromide). These medications may alter the safety and effectiveness of Itopride.

Overdose And Missed Dose

  • An overdose of Itopride may cause symptoms such as nausea, vomiting, abdominal cramping, and diarrhoea. In the event of overdose, contact your doctor or seek emergency medical attention immediately. These symptoms require prompt medical management. Treatment is symptomatic and supportive.
  • If you miss a dose of Itopride, take it before your next meal if you remember in time. If it is nearly time for your next scheduled dose, skip the missed dose and continue with your regular schedule. Do not take two doses at the same time to compensate for the missed one.

Management Of Side Effects

  • Take this medicine with food to reduce gastrointestinal side effects and continue taking it as prescribed. If side effects worsen or persist, consult your doctor for a possible treatment plan adjustment.

Use In Pregnancy And Lactation

  • Caution is advised while taking this medicine during pregnancy and breastfeeding.

Caution In Other Conditions

  • Caution is advised in older adults and patients with gastrointestinal bleeding, intestinal obstruction and perforations.

Special Dietary And Lifestyle Recommendation

  • Avoid alcohol, as drinking alcohol makes your stomach produce more acid than usual. It can irritate your stomach lining and make your symptoms worse.
  • Avoid rushing through meals, and take your time to enjoy and properly digest your food.
  • Eat smaller, frequent meals instead of large meals. It helps reduce the amount of food in the stomach, decreasing the likelihood of indigestion.
  • Inform your doctor if you do not experience an improvement in symptoms.
