Flupenthixol

Uses of Flupenthixol

Flupenthixol is an antipsychotic medicine used to manage symptoms of schizophrenia and other psychotic disorders. It may also be used at lower doses for the short-term management of depression.

Flupenthixol is commonly used for:

• Schizophrenia: Reduces symptoms such as hallucinations (hearing or seeing things that are not there), delusions (fixed false beliefs), disorganised thinking, and behavioural disturbances.
• Other psychotic disorders: Used in conditions that involve psychotic symptoms similar to those seen in schizophrenia.
• Depression with or without anxiety: At lower doses, Flupenthixol is sometimes prescribed for mild-to-moderate depression, particularly when anxiety is also present.
• Maintenance treatment: Used as a long-term treatment to prevent the return of psychotic episodes in people with schizophrenia who are stable.

Side effects of Flupenthixol

  • Tremors, muscle rigidity, and involuntary movements

  • Sedation and fatigue

  • Dry mouth and constipation

  • Dizziness and headaches

  • Weight gain

How Flupenthixol works

  • Flupenthixol works primarily by blocking dopamine receptors in the brain, reducing the abnormal dopamine activity that is associated with psychotic symptoms.
  • It is classified as a typical (first-generation) antipsychotic. Dopamine is a chemical messenger (neurotransmitter) in the brain that plays a key role in thoughts, mood, and perception. In psychotic conditions such as schizophrenia, dopamine activity is believed to be excessive in certain brain pathways, contributing to symptoms such as hallucinations and delusions.
  • Flupenthixol selectively blocks dopamine D2 receptors. By preventing dopamine from binding to these receptors, it reduces the overactive dopamine signalling responsible for positive symptoms of psychosis (hallucinations, delusions).
  • Flupenthixol also has effects on other neurotransmitter systems, which may contribute to its clinical effects.
  • At higher doses, the blockade of dopamine receptors provides antipsychotic benefit. At lower doses, Flupenthixol may have antidepressant effects in certain patients.

Expert Advice

Dosage Forms Available

  • Flupenthixol can be taken orally (tablet and oral solution) and administered intramuscularly (long-acting injectable depot formulations).

Age and Dose Restrictions

  • Flupenthixol is generally prescribed for adults and adolescents above the age of 12.

Contraindications

  • Flupenthixol is contraindicated in patients with known hypersensitivity to thioxanthene antipsychotics. It should not be administered to those suffering from severe CNS depression, blood dyscrasias, or severe hepatic impairment.

Other Medications to Avoid

  • Flupenthixol may interact with antihypertensives (e.g., beta-blockers such as atenolol or propanalol), diuretics (e.g., furosemide and toresimide), antiepileptics (e.g., carbamazepine and brivaracetam), and CYP2D6 inhibitors (e.g., fluoxetine, paroxetine).

Overdose and Missed Dose

  • An overdose of Flupenthixol may cause symptoms including excessive sedation or, conversely, agitation and restlessness, muscle stiffness, abnormal involuntary movements, low blood pressure, rapid heartbeat, and in severe cases, loss of consciousness. If you suspect an overdose, seek emergency medical attention immediately.
  • If you miss a dose of Flupenthixol tablets, take it as soon as you remember. If it is nearly time for your next dose, skip the missed dose and continue with your regular schedule. Do not take a double dose. If you miss a depot injection appointment, contact your doctor or clinic as soon as possible to reschedule. Consistent adherence to your prescribed treatment plan is important to maintain stability.

Management of Side Effects

Most side effects of Flupenthixol are temporary and generally harmless and resolve once the medication is discontinued. However, if you experience any severe side effects or worsening of any symptoms, please consult your doctor immediately.

  • Drinking plenty of water may help alleviate some common side effects like dry mouth and constipation.

  • Resting and avoiding heavy physical activity may help cope with drowsiness or fatigue caused by the medication.

Use in Driving and Operating Machinery

  • Flupenthixol may make you feel drowsy or dizzy, which could impair your ability to drive or operate heavy machinery. Refrain from such activities if you experience these side effects.

Use in Pregnancy and Lactation

  • The safety of Flupenthixol during pregnancy and lactation is not well established. It should only be used if the doctor suggests that the benefits outweigh the risks.

Caution in Other Conditions

  • Patients with pre-existing cardiovascular disease, seizure disorders, diabetes, glaucoma, or prostatic hypertrophy should exercise caution when taking Flupenthixol. Caution is advised for patients with pheochromocytoma. Regular monitoring and dose adjustments may be necessary in these cases.

Special Dietary and Lifestyle Recommendations

  • Ensure a consistent sleep schedule for adequate rest.
