Cefpirome

Uses of Cefpirome

Cefpirome is a fourth-generation cephalosporin antibiotic used in hospital settings to treat serious bacterial infections. It has a broad spectrum of activity against both Gram-positive and Gram-negative bacteria, including many strains resistant to earlier cephalosporins.

It is commonly used for:

Severe respiratory tract infections: Treats hospital-acquired pneumonia and other serious lung infections caused by susceptible bacteria.

Urinary tract infections (complicated): Manages serious kidney and bladder infections that require intravenous antibiotic treatment.

Skin and soft tissue infections: Treats deep wound infections and cellulitis caused by susceptible organisms.

Bloodstream infections (bacteraemia/septicaemia): Used in the management of serious bacterial infections in the blood requiring hospital-level care.

Intra-abdominal infections: Addresses severe abdominal infections, often alongside metronidazole for anaerobic coverage.

Febrile neutropenia: Used empirically (as a first-response treatment) in cancer patients with low white blood cell counts who develop fever.

How Cefpirome works

  • Cefpirome is a fourth-generation cephalosporin antibiotic. It works by disrupting the synthesis of the bacterial cell wall, which is essential for bacterial survival.
  • Cefpirome binds to specific proteins on the bacterial cell surface called penicillin-binding proteins (PBPs). These proteins are enzymes that help build and cross-link the peptidoglycan layer of the bacterial cell wall. By binding to and inhibiting these PBPs, Cefpirome prevents bacteria from forming a stable cell wall. Without this protective structure, the bacterial cell becomes vulnerable to osmotic pressure and eventually bursts and dies.
  • Cefpirome is resistant to degradation by many beta-lactamase enzymes that inactivate earlier cephalosporins, giving it enhanced activity against certain resistant bacteria including Pseudomonas aeruginosa and Enterobacteriaceae.

Expert Advice

Dosage Forms Available

  • Cefpirome can be administered via the intravenous route (injection).

Age and Dose Restrictions

  • Cefpirome is meant for adults and children over 12 years of age.

Contraindications

  • This medicine is contraindicated in patients with a known allergy to Cefpirome or any other cephalosporin antibiotics. It should also not be used by patients with severe renal impairment.

Other Medications to Avoid

  • Cefpirome may interact with aminoglycosides (such as gentamicin and neomycin), diuretics (like furosemide), and anticoagulants (such as warfarin).

Overdose and Missed Dose

  • Taking more than the prescribed dose of Cefpirome may cause symptoms such as seizures, severe nausea, vomiting, and diarrhoea. Cefpirome is given as an intravenous injection in a hospital setting, so accidental overdose outside this environment is unlikely. If a higher-than-intended dose is administered, medical staff will manage the situation with supportive care.
  • As Cefpirome is administered by healthcare professionals in a hospital or clinic, any missed dose will be managed by your care team. If you have concerns about a missed infusion, speak with the medical staff responsible for your care.

Management of Side Effects

Most side effects are temporary and generally harmless and resolve when discontinuing this medicine. However, if you experience any severe side effects or worsening of any of the symptoms, please consult your doctor.

  • Drinking plenty of water can help manage some gastrointestinal side effects like nausea and diarrhoea.

  • Resting and avoiding intense physical activity can help manage dizziness or fatigue.

Use in Driving and Operating Machinery

  • Cefpirome may cause dizziness or fatigue. If you experience these side effects, avoid driving or operating heavy machinery until you are sure you can perform these activities safely.

Use in Pregnancy and Lactation

  • It is recommended to consult your doctor before using Cefpirome during pregnancy and lactation. The potential benefits must be weighed against the potential risks before taking this medicine.

Caution in Other Conditions

  • Patients with kidney problems or a history of penicillin allergy should exercise caution when using Cefpirome. Regular kidney function tests may be required to monitor the effect of this medicine on the kidneys.

Special Dietary and Lifestyle Recommendations

  • Adequate hydration is essential for overall health and can help manage some side effects associated with Cefpirome.
